Whether you have a new concern, ongoing symptoms, questions about your health, or simply want help creating a plan, we take the time to understand what you need and determine the next best step.Â

Your initial 60-minute consultation gives us the opportunity to understand your concerns, review your health history and goals, and create an individualized plan. From there, you decide what level of care makes the most sense for you.

Single Acute Visit
If you only need care for one acute concern, you can simply complete that visit and return when you need us again.
Pay-Per-Visit Follow-Up
For patients who want continued care without a membership:

For patients who want an ongoing relationship with 513 Vitality, membership includes:

Up to 4 primary-care visits per month

In-person or telehealth visits for acute or chronic concerns
